Information for New Patients
Welcome to our practice! We appreciate the opportunity to be part of your oral healthcare team!
New patients are asked to print BOTH our Patient Registration & Health History and HIPAA for Patients forms found below and bring them to their first appointment. Please remember to bring your dental insurance card with you as well. You will be asked to provide any dental benefit information when making your appointment so that we can complete a complimentary benefits check before you arrive.
Whether you are seeing us for an All-on-4, LANAP Periolase Laser or Periodontal Evaluation, your first visit to Towson Center for Dental Implants and Periodontics will involve a few special steps so that we can get to know you. Before meeting your periodontist, a dental assistant will discuss and document your medical and dental history and review any medications you may be taking. We will also review any x-rays provided by your general dentist and take any additional digital images necessary.
Depending on your needs, you may need a second appointment called a Treatment Planning Consultation where detailed information regarding your personalized course of treatment and options are reviewed. Our Treatment Coordinators will guide you through the process and help answer any questions you may have. We encourage you to research our Digital Library covering a vast array of dental topics!
Please note, we occupy two suites in our building, suites 100 and 200. Initial consultations and surgical appointments take place in suite 200, hygiene appointments take place on the first floor in suite 100. For mobility challenged patients scheduled to be seen on the second floor, we offer two entrance options. A street-level ramp entrance can be accessed from the north-front side of our building and a lift is located through the courtyard on the lower level. Please let us know if you need assistance locating or entering our office.
Information for Surgery Patients
Patients coming to the office for a scheduled surgical procedure may refer to the pre-surgical and post-surgical information below before their appointment or refer to it at home during recovery. Your dedicated treatment coordinator will also review this information and any necessary prescriptions ahead of time. Do not hesitate to call us if you have questions, 410-321-9477.
Surgical patients taking any kind of prescribed sedative or scheduled to receive IV sedation are not permitted to drive to or from their appointment. Patients must be accompanied by a friend, family member, etc. and not rely on public transportation of any kind for these appointments.
